Incident Investigation & Root Cause Analysis (RCA)

This course delivers the analytical frameworks, forensic interviewing techniques, and evidence gathering protocols required to systematically investigate shipboard accidents, near-misses, and operational failures. Participants learn how to look beyond immediate human error to uncover latent systemic vulnerabilities using industry-standard methodologies such as the 5 Whys, Fishbone diagrams, and TapRooT. By focusing on timelines reconstruction, data preservation, and the formulation of corrective and preventive actions (CAPA), the curriculum equips senior officers and safety managers with the skills needed to eliminate recurring risks, foster an open “Just Culture,” and drive continuous improvement across fleet operations.
